During the registration process, a number of documents will be required. Descriptions of those documents can be found below.

Proof of Residency – current housing lease/deed/mortgage statement, current electric bill at your provided address, and a government-issued ID that matches this address. All provided documents must be in the name of the parent/legal guardian.

TN Certificate of Immunization or Exemption Form-Immunization exemptions:

A parent or guardian can file a written statement with the school if the child's religious beliefs conflict with the immunization.
A child can be excused from immunization for medical reasons if their doctor provides a written statement.
Students experiencing homelessness can enroll immediately, even if they cannot immediately provide immunization records.

Religious Exemption from Vaccination(s)

Proof of Physical – if transferring to Roane County Schools from out of state, you must provide us with proof of a physical exam within (30) days of enrollment. A physical exam done on/after January 1 of the current school year is acceptable. The physical exam must be notated on the Tennessee Immunization form.

Proof of Birth-one of the following forms:

Birth Certificate-issued by a government
Passport issued by any nation (translated)
Immigration documentation
Decree of adoption or other records issued by a court
Other official documentation showing date of birth and parents' names

Custody/Guardianship Papers – if applicable. These documents must be provided in the instance of divorce, legal separation, change of guardianship from parents to another party, etc.

*Photos of the above items may be taken with your phone or other mobile device to upload during registration. Apps such as Adobe Scan can be used to scan items from your phone into a PDF for upload, as well.

Below are the immunizations required for Pre-K and Kindergarten:

Pre-K Only:

HIB 3 or 4 doses depending on age of child at first immunization & type
Pneumococcal (PVC) < 5 years - 4 doses

Both Pre-K and Kindergarten

DPT, DTAP -- 4 or 5 doses (If only 4, consult nurse)
POLIO - 3 or 4 doses (if only 3, consult nurse)
HEP B 3 doses
HEP A 2 doses
Varicella 2 doses
MMR (Measles, Mumps, Rubella) 2 doses each
